On the 4th and 5th Of October GCE Group participated in the Healthcare Estates show in Manchester. This was a unique opportunity for the Healthcare team to showcase the new AVSU and Auto change manifold for the HTM marketplace.

Medical gas regulators are provided in many forms such as use at home flow selection devices to gas pipeline distribution devices in hospitals.
Applications, such as gas distribution inside an ambulance, only require pressure regulation. Regulators are preset for delivering a nominal pressure of 50 psi for FDA compliance, but there will be some variation in the outlet pressure depending on the consumed gas cylinder pressure.
If the gas source is connected to multiple devices, a high flow capacity regulator will be necessary.
Regardless of the type of regulator required, the materials are key consideration. Nickel-plated brass offers a good combination of durability, reliability and safety because brass is resistant to sparking, and has good compatibility with oxygen compared to other materials. A combination of material and design provides resilience to ignition by adiabatic compression, that's the phenomena where oxygen heats up due to a rapid change in flow/pressurisation. If this heating effect is sufficient and if there is contamination such as particles, then the energy may be sufficient to cause an ignition.

The purpose of this Study is to improve the design of devices and their respective instruction Manuals, based on the input of Users in England, UK.
Users will be given one of two portable models (Zen-O™ and Zen-O lite™) to familiarize themselves with, following which we will be asking them to perform certain basic tasks (e.g. turn the device on/off, check the battery level) and to give us their feedback based on a set of questions.
Since the findings of this Study will be utilized as input to the future designs of Oxygen Concentrators, we are keen to understand the Users' perspective.

Historically, regulating pressure and flow meant connecting a pressure regulator to the gas source and connecting a flow meter to the pressure regulator. This option increases complexity for ordering, inventory, training, cleaning and increased risk of leaks due to multiplicity of connections.
For facilities that want to streamline all these activities, especially in situations where a device may be used for different applications throughout its life, a combination regulator with flow selector device is the best choice.
The device such as that shown in the image illustrates the broad utilization of a single-stage regulator fitted with a flow selector. First, notice it has two outlet connections. The pressure outlet quick connector beneath the cylinder contents gauge supplies oxygen at a fixed 50 psi to medical devices such as a ventilator, CPAP or incubator.
The hose nipple outlet by the flow rate selector knob delivers oxygen flow to the patient devices such as a canula. The nipple can rotate 360 degrees to better position the tube in relation to the patient, and the outlet can be changed to allow for easy connection to a humidification device such as a nebulizer, as oxygen can dry the respiratory system.
The selector knob connects to a disc inside the regulator with 11 very small holes (e.g., 0.02 mm) around its perimeter. The size of the hole corresponds to a specific flow rate.
Depending on the model, flow ranges can be 0-2, 0-3, 0-6 or 0-25 l/m. Flow setting options vary by model. For example, the highest flow rate model has settings for 1, 2, 3, 4, 5, 6, 7, 9, 12, 15 or 25 l/m (the flow setting of 15 l/m allows for nebulization and 25 l/m rate allows for use with resuscitation).
For fine control, such as for neonatal care, a low flow rate model has settings for 0, 0.1, 0.2, 0.3, 0.4, 0.5, 0.6, 0.7, 0.8, 1.0, 1.5 or 2.0 l/m). This device uses a self-centering click system, avoiding settings in between two positions, and it provides a continuous flow between settings. In the unlikely event of indent mechanism failure, the patient will still be supplied with oxygen.
The correct oxygen regulation device depends on delivery method to the patient. Ventilators, anesthesia machines and other medical equipment have built-in flow control, so for those applications, only gas pressure regulation is necessary on the gas source.
In other applications, oxygen is delivered directly to the patient via a nasal cannula, oxygen mask, face tent, oxygen hood, CPAP, etc. These applications require a combination of pressure regulation and flow control.

How to clear up a common misunderstanding about the difference between gas pressure regulation and gas flow regulation?
Pressure is the measure of force applied on an area in pounds per square inch (psi). The gas source (a cylinder or bulk delivery system) is at a high pressure. For example, E size cylinders have service pressure of 1900 – 2,000 psi (note they may be filled up to 2,200 psi). A pressure regulator reduces the high service pressure to a much lower delivery pressure. In the US, 50 psi is a standard medical oxygen delivery pressure.
Flow is a measure of output in terms of volume per unit of time. In medical applications, oxygen flow is the dosage delivered to the patient and is expressed in liters per minute (l/m). Thus, flow meters and regulators serve distinct purposes!
Gas cylinders are identified using the letters A to M; cylinder volume increases as the letters of the alphabet proceeds.
For example, an H size cylinder holds about 6,900 liters.
Larger cylinders are most often used in hospitals and long-term care facilities as back up to the central gas systems. Smaller, more portable cylinders are commonly used to transport patients between rooms, for emergency medical services and homecare.
An E sized cylinder holds 660 - 690 liters (size varies slightly by provider) and is the most commonly used size in medical settings.

From 26th May 2021 the Regulation (EU) 2017/745 of the European Parliament and of the Council, on Medical Devices (MDR) is applicable and replaced the Council Directive 93/42/EEC, concerning medical devices (MDD).
With respect to transitional provisions GCE's EU Certificate according to Annex II of MDD is valid until 27th May 2024. This means that GCE can continue to place its Class IIa and IIb medical devices on the market until the certificate expires.
Class I medical devices have been assessed according to the new requirements and are in compliance with the MDR.

Yes, it is a chronic condition, but treatments are available to ensure that patients can maintain a healthy and somewhat active lifestyle. COPD affects more than 65 million people worldwide and is directly responsible for around three million deaths per year. The average five-year life expectancy ranges from 40 - 70 % - meaning that 40 - 70 out of every 100 people with COPD will survive longer than five years.

COPD is caused by long term lung damage as a result of breathing in harmful, airborne substances. This can include the inhalation of tobacco smoke, air pollution or other forms of toxic smoke. Prolonged exposure to chemicals, fumes and smoke in work environments can also cause COPD.
Only very rare genetic conditions can cause COPD, but if a parent has suffered from chest problems you are more at risk. However, you are most likely to develop COPD if you are a smoker, had a lung condition as a child and currently aged 35 years or older.

The basis of COPD treatment is to quit smoking. This is the only way to prevent further worsening of the disease and to maintain the ability to breathe.
Existing lung damage can no longer be repaired, so the goal of treatment is to relieve shortness of breath and coughing. In essence, helping maintain adequate breathing for as long as possible so that patients can perform everyday activities and lead a 'normal' lifestyle.
A doctor may prescribe medication, usually in an inhaler, to help open the patient's airways. They may recommend long-acting medicines that can be taken every day as well as short-acting medicines that are used when needed, such as before an activity.
In the event of an outbreak, other drugs are usually given in tablet form and hospital treatment may also be required. In more serious instances, oxygen therapy is required to manage COPD cases.
Supplemental oxygen therapy increases the amount of oxygen the lungs receive and deliver to the bloodstream. In the last few decades, great strides have been made to manufacture devices that patients with COPD can use, at home or on the move, to deliver this extra oxygen support.
So called Portable Oxygen Concentrators (POCs) are prescribed by medical professionals to patients with severe COPD. As a result of Covid-19, more and more patients, who are already in a stable state, are resorting to home solutions using oxygen therapy. This can lead to accidents and is potentially dangerous!
